How patients consent to our text messages

This page documents the exact opt-in experience for the Full Implant Choice text-message program, including screenshots of the live consent form.

Where the opt-in happens

Patients request a callback through the form on fullimplantchoice.com/contact. The form is rendered on that page inside an embedded frame served by our website platform, so automated crawlers may not see the form fields in the page's raw HTML — the screenshots below show what every visitor sees in a browser.

The form includes an optional SMS-consent checkbox that is unchecked by default and is not required to submit the form. Its full label reads:

I agree to receive texts from Full Implant Choice about my request. Msg & data rates may apply, frequency varies, reply STOP to opt out.” — followed by a link to our privacy policy.

Consent is stored with the submission, and text messages are only sent to patients whose submission has the box checked — this is enforced on our servers, not just in the form.

Other ways a text conversation starts

Patients who text the practice first receive conversational replies to their own inquiry. Patients who phone the practice to schedule are asked during the call whether we may text appointment confirmations and reminders to their number, and their answer is recorded with the appointment.

Every recipient can opt out at any time by replying STOP, or reply HELP for assistance. Message frequency varies; message and data rates may apply. Consent to receive texts is never a condition of receiving care.
